Please use the form below to report if you were omitted from a film or TV title, if you have any questions about your reported wages, or any other questions about your credited work.

PLEASE NOTE: A TV Film or TV Series ONLY qualifies for an FMSMF payment if it was produced AFTER 7/1/71 AND is exhibited in supplemental markets (i.e. free TV, pay-cable TV, DVD, New Media, etc.).
