Important Information About Your
2002 Tax Form

The New Year is fast approaching and with that comes tax time. In January of 2003, the Film Musicians Secondary Markets Fund (formerly known as the Theatrical & Television Motion Picture Special Payments Fund) will be sending out W-2 and 1099 forms for all U.S.–resident participants who had earnings in 2002 in addition to RL-1 and T-4 forms for our Canadian–resident participants.

If your address information has changed since your last check was issued, it is very important that you inform the Fund of your current address information by January 5th, 2003.

Simply do one of the following:

• Call and request a Musician’s Confirmation Form be sent to you either via mail or fax.
  
  
  
• Visit our website at www.fmsmf.org and update your address online. (Note: This option is available only to those participants who have already signed up for the Private Area of our website)
  
  
  
• Send a written document containing your updated address information to the Fund office either via mail or fax. (Important: This documentation is only valid with the participant’s signature and the date)
