W-4 Reminder


Important W-4 Reminder Notice:  To all Musicians

 

Since you last filed Form W-4 with the Film Musicians Secondary Markets Fund office did you...

        Marry or divorce?
        Gain or lose a dependent?
        Change your name?

Were there major changes to your:

  • Non-wage income (interest, dividends, capital gains, etc.?)
  • Family wage income (you or your spouse started or ended a job?)
  • Itemized deductions?
  • Tax credits?

If you answered yes to any of these questions you may need to file a new Form W-4.   If the number of withholding allowances that you are entitled to increases or decreases, you may submit a new W-4 any time thereafter.  

Please call the Fund’s Participant Services Help hotline at (818) 623-4300 if you need to know your current tax profile or you want a new W-4 form mailed to you. A new W-4 form may be also completed in the FMSMF office. Please call (818) 623-4300 to SCHEDULE AN APPOINTMENT if you would like to complete a new W-4 form at our office.

Please review your W-4 status and file a new W-4 form if your filing status or number of withholding allowances will change in the coming year.  To help ensure that you do not have too much or too little income tax withheld from your residuals payments, you may also go to the “Withholding Calculator” in the “Individuals” section of the IRS Website at:
