This form should be used to report crediting problems (you were omitted from a film, your reported wage is incorrect), name, address changes, etc., or to ask questions about a specific film or television production. If none of these apply, you do not need to fill out this form. For new-use (pre-recorded music used in a film) please include song title, artist and original recording location and date in the Comments field on page two of this form.

Please Note: A TV Film or TV Series ONLY qualifies for a FMSMF payment if it was produced AFTER 7/1/71 AND is exhibited in supplemental markets (i.e. free TV, videocassette/DVD and/or pay-cable TV).
