If you would like to designate a beneficiary or update your current
beneficiary information, please print out the request form below in either HTML, PDF or MS Word formats and either fax or mail to the Film Musicians Secondary Markets Fund.

Please fill out the form completely.  You must include your printed & signed name, the date, your mailing address, phone #, and last four digits of your Social Security Number or Fund Account number.  Once we recieve your completed request, a new beneficiary designation card will be mailed out to you.
