If you would like to designate a beneficiary or update your current
beneficiary information, or find out who you have designated, please print out the request form below in either HTML, PDF or MS Word formats and either fax or mail to the Film Musicians Secondary Markets Fund.

Please fill out the form completely.  You must include your printed & signed name, the date, your mailing address, phone #, and last four digits of your Social Security Number or Fund Account number.  Once we receive your completed request, a call will be made to you informing you of your designation(s) and/or a new beneficiary designation card will be mailed out.

If you would like to see your current beneficiary designation(s), please refer to your latest online musician’s statement located on the Private Area Main Page under Annual Distribution Statements.  If you feel you have updated your designation(s) since our last distribution, and you would like to know whether that update was made, please follow the instructions above.

Note: If you see your own name listed as a beneficiary (primary/alternate), it just means you have designated your trust/estate as a beneficiary.
