The “Information Update” deadline is quickly approaching with regards to your FMSMF distribution. Changes regarding ADDRESS INFORMATION, DIRECT DEPOSIT, TAX WITHHOLDING, and/or MARITAL STATUS must be completed by JUNE 1ST. This will ensure that your information is updated in plenty of time for the annual mailing.

If you are registered for online registration, many of these changes can be made via the Private Area Members Section of the Fund’s website. We estimate that the final statements will be available online 7 to 10 days prior to the July 1st distribution.

If you have not done online registration, FMSMF recommends that you take the time to do so. In addition to being able to update your personal information, you can print current and prior statements. Tax Forms (W-2's, 1099's and T4A's) are no longer available online but can easily be obtained by contacting the Fund’s Participant Services Department at 818-623-4300 or via e-mail at participantservices@fmsmf.org.
Registered participants may take advantage of the Fund’s “Go Paperless” option. This option requires that you have Direct Deposit.

Some changes, which require back-up and/or signatures, must be done either by snail-mail, fax, or in person. Participants who may be preparing to file for divorce should contact the Fund as soon as possible for information on what is required. Please check the website for all available information. By keeping your information current with the Fund, you save administrative costs which all participants pay for. Reviewing your status is the best way to ensure that your residual payment arrives on time and in the manner most secure and convenient for you!
